Your technician has determined that a replacement device should be sent to you before your broken device is received by LoneStar Tracking. This form serves as acknowledgement, that you understand that if we do not receive your broken device within 14 days, your credit card will be automatically charged the full amount of a replacement device. As well, if your device is received in a condition other than “like new”, you will be charged for a new device. ONLY proceed with this form if you agree to these terms.
